Welcome to Spring Pointe Memory Care Community! Memory Care At Spring Pointe we offer a safe and secure environment for those with Alzheimer's and other dementia-related illnesses. Our unique program provides a quality-of-life that recognizes and respects dignity while encouraging social interactions. Our goal for every resident is to maximize their independence while providing assistance as needed. Memory Care Services include: Medication management Activity therapy Safety and security checks Personal laundry and housekeeping Program Coordinator Eating assistance/special diets 24-hour supervision We have taken every step to understand Alzheimer's Disease and the impact our special community will have on the enhancement of each resident's quality-of-life. We are a close-knit family and you can be assured of the personal attention that your loved one will receive. The staff at Spring Pointe understands the needs of our residents, and through training and compassion, lovingly cares for their needs. Philosophy of Care Spring Pointe Memory Care Community is dedicated to promoting the social, psychological, spiritual, and physical well being of all residents. Spring Pointe seeks to provide the highest quality of life and to make a positive contribution to the larger community of which we are a part. We believe in the dignity of each individual, recognizing that every person has physical, mental, social, emotional, and spiritual needs and rights, and that these rights must be respected. Our intent is to maximize the capabilities and allow residents to function as independently as they are able, while providing assistance as needed. Our Environment We have developed a unique environment for a unique approach. Our community looks and feels just like home. Activity rooms and dining rooms are strategically located to encourage social interaction and involvement. We have created a balance of privacy and community. We have continuous walkways indoors and outdoors. The colors, lighting, and patterns are soothing to our residents, and our furniture has all the comforts of home. Activity Therapy Our activity therapy program is designed to meet the needs of each resident. Each program is developed specific to habits, interests, and past history. This enables residents to enjoy the highest quality of life. Community activities are scheduled throughout the day to promote cognitive, sensory, and physical skills. In addition, we have developed our program to provide one-on-one individual activities that enhance each resident's self care and independence with every day living tasks. Our goal is to create rewarding experiences for the resident, family, and staff. This stimulating environment is created to encourage residents to participate in the daily routines of a home-like setting. Basic Services offered to all Spring Pointe residents include: 24-hour on-site staff 3 nutritious meals served daily Life enhancing activities Coordination of health resources Base level assistance with activities of daily living Weekly housekeeping Weekly linen service Apartment maintenance All utilities (except telephone & cable) Monitoring of resident's health status Personal Assistance Our goal at Spring Pointe is to meet each resident's needs while maintaining the highest level of choice, dignity, privacy, respect, and individuality. To assure that this is done effectively, an assessment is conducted prior to move-in. Residents, family members, and Spring Pointe staff work together to develop an individually-tailored Assistance Plan. A follow-up family care conference will be held regularly to assure that, as a resident's needs change, the Assistance Plan is adjusted accordingly. In addition to Basic Services, Spring Pointe offers Personal Assistance Plans for those who need additional help with their daily routines. Some of these services include: Select diets or snacks Daily assistance with ADL's Assistance with ambulation to and from meals and activities Incontinence care Additional housekeeping services Medication management: monitoring, supervision, or assistance Safety and security checks Meal time reminders Medical appointment scheduling Periodic nighttime care Select diabetic care Health services: monitoring, supervision, or assistance Rental Information Apartments are rented month-to-month. No buy-in is required.
